Our collaborative Care Teams, which include older adults, caregivers, healthcare professionals, and specialists, ensure you receive the holistic support you deserve. Our Integrated Health Plans cater to older adults aged 65 and above, individuals under 65 who require assistance to maintain their independence, and members utilizing our Adult Family Care, Personal Care Assistance, and Home-Delivered Meals services.
For adults under 65 with both MassHealth and Medicare, One Care provides comprehensive coverage, encompassing vital in-home and community support services. Our dedicated Long-Term Support Services Coordinators (LTSCs) work closely with One Care plans to deliver personalized care coordination and facilitate access to essential resources. We proudly collaborate with esteemed One Care Teams, including those from Commonwealth Care Alliance and United Healthcare.
We are committed to providing customized care management and coordination to MassHealth ACO members with intricate LTSS needs through our partnership with LTSS Community Partners. By collaborating closely with Accountable Care Organizations (ACOs) and Primary Care teams, we continuously work towards elevating the quality of care for each individual under our service. We work with members on their goals regarding services, supports and health related social needs.
