Our Nutrition Program offers home-delivered meals for lunch and dinner on a daily or weekly schedule, with a variety of nutritious options. We also provide meals at senior centers and housing complexes, along with personalized nutrition counseling and support from our Registered Dietitian.
Our Home Care Care Managers and RNs support older adults, individuals with disabilities, and their caregivers by creating personalized care plans and connecting them to essential resources.
Adult Family Care (AFC) supports individuals 16+ needing help with daily activities by providing care in a family-style home. Caregivers receive payment for offering assistance, meals, transportation, and a room.
Personal Care Management helps you to choose and manage a Personal Care Attendant (PCA). PCAs can be family or friends, and assist with daily tasks like dressing, bathing, cooking, and medication management.
