Access Care Partners, Inc, a designated Area Agency on Aging and Aging Services Access Point, was founded in 1974 to serve older individuals, persons with disabilities and family caregivers. Based in Holyoke, MA, our Planning and Service Area (PSA) includes the following communities: Belchertown, Chicopee, Granby, Holyoke, Ludlow, South Hadley and Ware.
In its Area Agency on Aging role, Access Care Partners develops an Area Plan every four years to provide a “blueprint” of strategic goals and objectives that correspond to the guidelines required by the U.S. Administration for Community Living (ACL) and the Massachusetts Executive Office of Aging and Independence (AGE).
The Administration for Community Living (ACL) has identified the following key Focus Areas which must be addressed in all State and Area Plans on Aging across the country:
Access Care Partners’ Area Plan includes strategic goals and objectives which align with the ACL Focus Areas and prioritize services which meet the needs of the OAA target population: those individuals with the greatest economic and social need, including low-income and isolated individuals.
Access Care Partners is a private, non-profit agency founded in 1974 which is funded in parts by grants and contracts from the Massachusetts Executive Office of Aging & Independence (AGE) and the federal Administration for Community Living. We are part of a network of Aging Services Access Points and Area Agencies on Aging.
