At Access Care Partners, we know that good nutrition is essential for overall well-being, particularly for older adults and individuals with disabilities. That’s why our Nutrition Program goes beyond just delivering meals—it’s about fostering health, safety, and community connections.
More Than a Meal: The Power of Home-Delivered Nutrition
For many older adults and individuals with disabilities, preparing nutritious meals can be challenging due to mobility limitations, financial constraints, or health concerns. Access Care Partners’ home-delivered meals program, also known as Meals on Wheels, ensures that our consumers receive balanced, ready-to-eat meals right at their doorstep. Medically tailored, vegetarian or Latino meals options are offered as well.
But our program is much more than a meal. Each delivery includes a daily wellness check performed by our dedicated Nutrition drivers, who are trained to look out for signs of distress or changes in well-being. In fact, 38% of our nutrition consumers live alone, relying solely on their meal drivers for crucial wellness checks. These checks often identify concerning changes in physical or mental health, allowing for prompt action to ensure the safety of our consumers while providing peace of mind for caregivers.
Beyond nutrition and safety, our meal deliveries help combat social isolation. 80% of meal recipients report feeling less lonely due to their interactions with their meal drivers, demonstrating the program’s significant role in reducing loneliness and fostering human connection.
For many, these meals are a lifeline. 79% of consumers regard their Home-Delivered Meal as their main meal of the day, highlighting the crucial role our service plays in meeting daily nutritional needs. Additionally, 85% of consumers credit receiving meals with helping them live independently, underscoring our program’s impact in enabling older adults and individuals with disabilities to maintain their autonomy and dignity.
The benefits extend beyond just nourishment—88% of consumers report feeling better as a result of receiving Home-Delivered Meals. Among them:
All home-delivered meals are prepared locally by Delaney’s Market, following the nutrition and portion guidelines from the Massachusetts Executive Office of Aging & Independence. This ensures that each meal is both nutritious and tailored to the needs of older adults.
Gathering for a Meal: The Community Table
For those who are able to get out and about, our Community Table program provides nutritious congregate-style meals at local senior centers. Available at the Granby and South Hadley Senior Centers, these meals are served in a welcoming environment where older adults can share a meal, engage in conversation, and form meaningful connections with peers. Social engagement is a key component of healthy aging, and our Community Table sites offer the perfect setting for both nourishment and companionship.
Like our home-delivered meals, Community Table meals are also prepared by Delaney’s Market, ensuring they meet the same high standards for nutrition and portion control.
Honoring Cultural Traditions: The Latino Restaurant Community Table Program
Access Care Partners is also committed to serving diverse communities through our Latino Restaurant Community Table Program. In partnership with Taino Restaurant in Holyoke, we offer authentic Latino meals tailored to the tastes and traditions of the local Latino community. Older adults can enjoy flavorful, culturally familiar dishes for a suggested donation, ensuring that financial limitations do not stand in the way of a nourishing and enjoyable meal. This program not only provides essential nutrition but also celebrates the rich culinary heritage of our community.
