Our Story

For five decades, nestled in the heart of Holyoke, MA and serving Western MA, Access Care Partners has been a dedicated ally and advocate for older adults and people with disabilities and their caregivers. Our story began in 1974, first as Holyoke/Chicopee Senior Service, then under the banner of WestMass ElderCare, with a mission to enhance lives by empowering individuals to live safely and independently in their homes and communities. After 50 years, we embrace a new chapter in 2025 by becoming Access Care Partners, a name that truly represents our mission and vision.

At Access Care Partners, there is no one-size-fits-all solution. Our personalized solutions reflect our care recipients’ and their caregivers’ physical and emotional needs, living conditions, cultural backgrounds, lifestyles, relationships, and communities. Our team navigates the maze of resources to find the services, providers, supports and adaptive equipment that match our care recipients’ needs. If we don’t offer these resources ourselves, we connect our consumers with providers who do. The results are “best fit” solutions that preserve the dignity, independence, and quality of life of older adults and persons with disabilities and chronic conditions and their caregivers.

Access Care Partners, formerly WestMass ElderCare, is a private, nonprofit agency designated by the Massachusetts Executive Office of Aging and Independence (EOAI) as an Aging Services Access Point and an Area Agency on Aging, and contracts with EOAI for a number of programs.

Access Care Partners  has a strong network of relationships with local and statewide organizations working to assist older adults and persons with disabilities to maintain independence at home and to support caregivers. Access Care Partners contracts with more than 40 provider agencies for services such as personal care, homemaking, home health aide, medication dispensing devices, transportation, chore service, adult day health, and personal emergency response devices.

In addition, Access Care Partners contracts with MassHealth to offer Adult Family Care, Personal Care Management and LTSS Community Partners Programs. WMEC also partners with Senior Care Options (SCO), One Care, and PACE Programs to provide long term support services. SCO contracts are held with Commonwealth Care Alliance, United Healthcare, Fallon Navicare, Point 32 Health/Tufts, Senior Whole Health/Molina and Wellsense.

Access Care Partners engages community volunteers to serve on its Board of Directors, Citizens Advisory Council and Money Management Advisory Council. These groups plan and advise on issues affecting older adults and persons with disabilities in our region by guiding the work of Access Care Partners.
