For five decades, nestled in the heart of Holyoke, MA and serving Western MA, Access Care Partners has been a dedicated ally and advocate for older adults and people with disabilities and their caregivers. Our story began in 1974, first as Holyoke/Chicopee Senior Service, then under the banner of WestMass ElderCare, with a mission to enhance lives by empowering individuals to live safely and independently in their homes and communities. After 50 years, we embrace a new chapter in 2025 by becoming Access Care Partners, a name that truly represents our mission and vision.
At Access Care Partners, there is no one-size-fits-all solution. Our personalized solutions reflect our care recipients’ and their caregivers’ physical and emotional needs, living conditions, cultural backgrounds, lifestyles, relationships, and communities. Our team navigates the maze of resources to find the services, providers, supports and adaptive equipment that match our care recipients’ needs. If we don’t offer these resources ourselves, we connect our consumers with providers who do. The results are “best fit” solutions that preserve the dignity, independence, and quality of life of older adults and persons with disabilities and chronic conditions and their caregivers.
Access Care Partners is a private, non-profit agency founded in 1974 which is funded in parts by grants and contracts from the Massachusetts Executive Office of Aging and Independence. We are part of a network of Aging Services Access Points and Area Agencies on Aging.
