GUIDE is a new Medicare program designed to help people with dementia, and their family caregivers get the support they need to stay in their homes safely.

GUIDE Participants are paired with a personal care navigator who can connect them with local community resources such as home-delivered meals and transportation to medical appointments. Family caregivers receive customized education on supporting a person living with dementia and may qualify for a yearly allowance of respite services, giving them a break to recharge.

Care navigators are available 24 hours a day to answer questions, coordinate medical appointments, or review care plans with participants to ensure they meet their needs.

GUIDE Program Highlights

  • Services are covered at no additional cost to the client or caregiver.
  • $2,625 annually for in-home or adult day center respite (time off for the caregiver).

Who Qualifies

Who Qualifies?
• If you/your loved one has a diagnosis of mild, moderate, or severe dementia you may qualify.
• You have traditional Medicare parts A&B (not enrolled in Medicare Advantage).
• You are not enrolled in a SCO/One Care, PACE or hospice.
