Caring for a loved one can be both rewarding and challenging. At Access Care Partners, we understand the dedication and love you pour into caregiving every day. That’s why we’re here to offer coordinated support and practical resources to help you navigate this journey with confidence and compassion.

Through our Family Caregiver Program, caregivers can access education, support groups, respite options, and guidance to help ease stress and prevent burnout.  Read more below about this program.

For caregivers who provide daily, hands-on support to an older adult or person with a disability living in their home, our Adult Family Care (AFC) program offers additional support, including nursing oversight, case management, and a monthly stipend for eligible caregivers. We also offer Personal Care Management services for individuals who need help managing personal care needs while continuing to live safely at home. We can help you hire and manage a Personal Care Attendant, giving you greater flexibility and control over your care.  Find out more about the PCM program here.

Together, these programs are designed to support both caregivers and those they care for—because caregiving works best when no one has to do it alone.

Ways We Can Help

Who Do We Support?

  • Older adults aged 60 or above who are providing care. 
  • Individuals of any age caring for a loved one with Alzheimer’s Disease or related dementia. 
  • Grandparents or relative caregivers aged 55 or older, raising grandchildren under 18. 
  • Adult children with disabilities, aged 19 to 59, and their caregivers. 
  • Caregivers of all ages who need a well-deserved break and respite care.
