Empowering Individuals to Take Control of Their Health: New Virtual Workshop Series Begins July 16

Holyoke, MA — Access Care Partners (formerly WestMass ElderCare), in collaboration with Highland Valley Elder Services, is excited to announce a new virtual offering of Taking Charge: Living Well with Chronic Conditions, a free 7-week Chronic Disease Self-Management Program (CDSMP) beginning Wednesday, July 16, 2025, and continuing weekly through August 27, 2025, from 10:00 AM to 12:30 PM.

This empowering, evidence-based workshop—originally developed by Stanford University—is designed for individuals living with chronic health conditions such as diabetes, arthritis, high blood pressure, and other long-term health concerns. It is also an excellent opportunity for caregivers seeking practical tools to better support their loved ones.

Participants will gain actionable skills and confidence to:

  • Manage common symptoms such as pain, fatigue, and stress
  • Improve nutrition and increase physical activity
  • Communicate effectively with healthcare providers
  • Set achievable goals and maintain motivation

This program is about more than just information—it’s about transformation,” said Sarah Aasheim, Director of Community Programs of Access Care Partners. “It’s designed to help people feel empowered to take charge of their health and live their best lives—even in the face of chronic conditions.

Sessions will be led by experienced facilitators:

  • Andi Waisman, M.S.Ed., Educator Consultant
  • Teagan Swanson, CRS/A-D, Caregiver Specialist, Highland Valley Elder Services

The workshop is free and offered entirely online, allowing participants to join from the comfort and safety of their homes.
